Insurance Information

The insurance networks I participate in are:  BCBS IL PPO and Blue Choice PPO.  If you have a different insurance, I’ll provide you with an “insurance-ready” receipt at the end of each month so that you can submit to your provider.  

I encourage you to call the member services phone number on the back of your insurance card and ask:

1) What are your benefits for an out-of-network LCSW behavioral health provider?

2) What is your out-of-network deductible?

3) Once that deductible has been met, what percentage does your insurance reimburse?

4) How many visits are you allowed/calendar year?

5) Is there a difference for in-person vs. telehealth?
